Download, print, sign, and mail your authorization card.

Signing authorizes the Guild to petition the National Mediation Board to hold a representation election. It is not the election and it is not a vote — it is the act of embracing the change, and it takes minutes.

Sign and send — step by step

Eight steps, a few minutes, one envelope.

    Download and print the card

    Open the approved card and print it on plain white paper. Use only this approved card — do not change any of its wording.

  2. 2

    Print your full name

    Write your full name clearly on the "Print" line.

  3. 3

    Write your base or domicile

    Fill in your base or domicile on the "Base/Domicile" line.

  4. 4

    Write your email address

    Fill in your email address on the "Email" line.

  5. 5

    Write today's date

    Fill in the date on the "Date" line. The card must be dated in your own handwriting.

  6. 6

    Sign it by hand, in ink

    Sign the "Sign" line with a pen, in your own handwriting. Typed, digital, or electronic signatures cannot be accepted.

  7. 7

    Seal it in an envelope

    Place the signed card in a plain envelope and seal it. Your right to sign this confidential card is protected by law — your employer will never know if you signed.

  8. 8

    Place a stamp on it and mail it to the Guild

    Put a stamp on the sealed envelope and mail it to the address below, or hand it directly to a Guild member.
